*HMO Plans require that you select a Primary Care Provider (PCP). If The Doctor Is In is not your PCP, payment in full is required at the time of the visit.
Even though your plan may be listed here, it is not a guarantee of your coverage. If you have any questions regarding details and/or restrictions of your plan, please contact the member services phone number listed on your insurance card.
Please bring the following with you at every visit: insurance card, photo identification, and co-pay.
Some carriers limit the services we can perform in our office. If you require one of those services, you will be given the choice of going to an outside facility or waiving your insurance benefit, paying in full and having the service here.
